コロナワクチンの予防接種の説明 および予約
7月2日16:00より世田谷区よりクリニックに配布されるワクチンが、
20バイアル（120名）/2週間に制限されました。
これに伴い新規の接種受付を大幅に制限することになりました。
大変申し訳ありませんが、ご了解ください。

1 Webのみでの予約となります
（6月30日より区システムに移行いたしました）
- The reservation is only available on the Web.
2 来院の際は、必ず接種券、保険証をご持参ください。保険証のない方は接種をいたしません。
- Please bring the coupon and your health insurance card.
- If you do not have your health care card, you will never be vaccinated.
3 その他の受診予約からでは接種はできません
- We do not accept reservations in the doctor's name.
